What services are bundled into CPT 77301 (IMRT Planning)?
Under CMS guidelines, CPT 77301 includes the initial 3D simulation (77295), basic dosimetry (77300), and treatment devices (77334) when performed for the initial plan. Physics consultations (77336) and port verification films (77417) remain separately payable during the treatment course.
How do you calculate weekly radiation treatment management (CPT 77427)?
CPT 77427 is reported once for every 5 fractions delivered. At the end of treatment, a remaining cluster of 3 or 4 fractions qualifies for an additional 77427 billing; however, 1 or 2 remaining fractions are non-billable standalone under CMS § 100.1.
