No. CPT 94060 (bronchodilator responsiveness evaluation) encompasses both the pre-bronchodilator baseline spirometry and the post-bronchodilator repeat testing. Billed together on the same date of service, CPT 94010 is an NCCI column 2 code to 94060 and cannot be unbundled with any modifier.
What documentation is required to overturn CFTR modulator prior authorization rejections?
Appeals must include certified CLIA molecular genetic sequencing demonstrating at least one F508del mutation (or FDA-approved responsive CFTR variant), baseline percent predicted FEV1 spirometry curves, comprehensive liver function panels, sweat chloride quantitative baseline concentration (>= 60 mmol/L), and an attestation of CF Foundation accredited center multidisciplinary oversight.
